At the heart of Daylesford Crossing is Connections Memory Care, a specialized program that compassionately meets the needs of those living with Alzheimer’s or other forms of dementia. This positive, holistic approach recognizes each individual is unique, with needs that are ever-changing; day to day, even hour by hour.

Connections Memory Care residents receive support that empowers their potential. Trained professionals take the time to get to know each resident, understand their needs, respect their choices and preferences, and help them make the most of every day. In addition, we have eliminated the worry over unpredictable cost of care, with our all-inclusive bundled care—you pay one fee for all your loved one’s care needs.*

Positive Approach to Care®

Through a program called Positive Approach to Care® developed by internationally acclaimed dementia care expert Teepa Snow, our Memory Care team identifies each individual’s changing abilities and provides a personalized daily routine. This creates a predictable, soothing environment in which residents can feel more secure, calm, and comfortable.

Our Connections neighborhood centers around a central, large courtyard, providing comfortable and soothing surroundings with large windows that invite the outdoors in. A beautiful, restorative ambiance where residents feel at home, with attentive staff on site 24/7 for support and safety.

A Variety of Floor Plans

Personal taste is honored and respected in our wide array of Memory Care apartment options. Spacious and purposely designed studios and one- or two-bedroom floor plans are equipped with important safety features, which include oversized bathrooms offering wheelchair accessibility through doorways and into showers, as well as safety bars. Extra-large windows and high ceilings offer an open, airy feel throughout the residence.

Normal forgetfulness is part of the aging process and usually begins in middle age. However, there is a great difference between occasionally forgetting your keys and dementia. People with dementia cannot retrieve information and in most cases, lose the ability of how to remember. Dementia can be a variety of diagnoses. The most common is Alzheimer’s, a severe, progressive loss of recent memory and thinking ability.

A few factors to watch for: a senior who suddenly stops doing a longtime hobby or withdraws. Sudden weight loss. Change in grooming habits. A house in disorder. An inability to follow conversations. If you suspect that your loved one’s memory loss is disrupting their daily routine, it could be time to seek medical advice.
